Personal Information Personal information is collected by Eudamonia Health Services only when you specifically and knowingly choose to provide it. Information regarding services received by a health consultant is collected as part of the normal course of business, but this information is not disclosed to third parties.
COLLECTION AND USE OF PERSONAL INFORMATION FROM HEALTH CARE PROVIDERS Eudamonia Health Services may also contact you from time to time to conduct market research and surveys in an effort to improve our product and service offerings. Unless you specifically authorize us to release your personal information, a release is required or permitted by law, Eudamonia Health Services will not sell, lease or trade your personal information to other third parties. Anonymous or "non-identifiable" information gathered by the Eudamonia Health Services may be used for technical, research and analytical purposes. Information collected through surveys may be used by Eudamonia Health Services to analyze its market and to develop or enhance service offerings. Eudamonia Health Services will only ask for information about you that we need and will only ask for it when we need it. When we ask you for the information, we will explain to you why we need it and what we are going to do with it. We will seek your prior consent to the collection, use and disclosure of the information as required by applicable privacy legislation. Subject to legal and contractual requirements, you can refuse to consent to our collection, use or disclosure of information about you. You may also withdraw your consent to any further collection, use or disclosure of information about you at any time by giving us reasonable notice. If our intended disclosures or uses of information change, we will notify you accordingly. Your personal information will not be used for any other purpose without your consent.

Anonymous or "non-identifiable" information gathered by Eudamonia Health Services may be used for technical, research and analytical purposes. Information collected through surveys may be used by Eudamonia Health Services for marketing research purposes. Eudamonia Health Services will only ask for information about you that we need and will only ask for it when we need it. In order for a test to be processed, the requisition must have at a minimum: name, birth-date, time of saliva collection, and date of saliva collection. We also require information regarding menstrual status and hormone therapies in order to assign the correct range for the hormone levels. Symptom information is optional, however the information you provide enables the Medical Director to provide an interpretation of your results. If the symptom information is not provided, a minimal interpretation will be given. Subject to legal and contractual requirements, you can refuse to consent to our collection, use or disclosure of information about you. You may also withdraw your consent to any further collection, use or disclosure of information about you at any time by giving us reasonable notice. If our intended disclosures or uses of information change, we will notify you accordingly. Your personal information will not be used for any other purpose without your consent.
ACCURACY
SECURITY
ACCESS Eudamonia Health Services will only refuse access to information about you in those circumstances permitted or required by applicable privacy legislation. If Eudamonia Health Services refuses access to you, it will provide you with the reasons for its refusal upon request. Exceptions may include information that contains references to other individuals, information that cannot be disclosed for legal, security or commercial proprietary reasons, and information that is subject to solicitor-client or litigation privilege. Eudamonia Health Services will respond to your requests for access in accordance with applicable privacy legislation.
QUESTIONS OR CONCERNS If you are not satisfied with the response from Eudamonia Health Services after making a complaint, you may have recourse to additional remedies under applicable privacy legislation. For further information, please contact the federal Privacy Commissioner or your provincial Privacy Commissioner, as applicable. |
